Under normal conditions, semaglutide swallowed as a pill would be destroyed by stomach acid and digestive enzymes before it ever reached the bloodstream.
Novo Nordisk's solution is a co-formulation with sodium N-[8-(2-hydroxybenzoyl) amino] caprylate, known as SNAC. SNAC is an absorption enhancer that performs two functions. It temporarily raises the local pH around the tablet, creating a small buffer zone that protects semaglutide from acid degradation. It also promotes transcellular absorption, helping the semaglutide molecule pass through the stomach lining and into the bloodstream. What the Research Shows

OASIS 1 enrolled 667 adults with obesity or overweight (BMI of 30 or above, or 27 or above with at least one comorbidity) who did not have diabetes. Over 68 weeks, oral semaglutide 50 mg produced an average weight loss of 15.1% of body weight, compared to 2.4% with placebo (PMID: 37385275). That 15.1% figure is nearly identical to the 14.9% achieved with injectable semaglutide 2.4 mg in the STEP 1 trial. For the first time, an oral formulation matched injectable efficacy for clinically significant weight loss.
OASIS 2 tested the same doses in adults with Type 2 diabetes and obesity. Oral semaglutide 50 mg produced weight loss of approximately 10% at 68 weeks, roughly in line with what injectable semaglutide 2.4 mg achieved in the STEP 2 trial for the same population. The consistent pattern across both trials: at sufficiently high oral doses, the weight loss gap between oral and injectable semaglutide effectively closes.
However, there is an important caveat. The OASIS trials used higher oral doses than anything currently approved. As of early 2026, the only FDA-approved oral semaglutide product is Rybelsus, available in 3 mg, 7 mg, and 14 mg doses, approved for Type 2 diabetes only. Novo Nordisk has submitted regulatory applications for higher-dose oral semaglutide for weight management, but these have not yet received FDA approval. Until they do, patients who want oral semaglutide are limited to the 14 mg ceiling, which produces significantly less weight loss than injectable semaglutide at 2.4 mg.

A patient who has maxed out oral semaglutide at 14 mg and wants the weight loss efficacy demonstrated in the STEP trials would need to switch to injectable semaglutide. That transition requires medical supervision because oral and injectable doses are not directly interchangeable. The dose conversion is not a simple ratio due to the bioavailability difference. The OASIS data suggests that oral semaglutide at those doses can match injectable efficacy. A pill that produces 15% weight loss without needles would be a meaningful advance for patients who are unwilling or unable to self-inject. Until that approval happens, the choice between oral and injectable semaglutide is really a choice between convenience and efficacy, and the gap is significant.
