How They Work

Cagrilintide is a long-acting analog of amylin, a hormone co-secreted with insulin by the beta cells of the pancreas. In a healthy body, amylin does three things after a meal: it slows gastric emptying (independently of GLP-1), it suppresses glucagon secretion (which prevents your liver from dumping glucose after you eat), and it signals satiety through the area postrema and other brainstem nuclei. This last point is critical. Amylin acts on satiety centers in the hindbrain that are anatomically distinct from the hypothalamic pathways targeted by GLP-1. Tirzepatide takes a different dual-pathway approach. Rather than combining two separate drugs, tirzepatide is a single engineered molecule that simultaneously activates both GLP-1 and GIP (glucose-dependent insulinotropic polypeptide) receptors. GIP is the other major incretin hormone. Its receptors exist in fat tissue, pancreas, brain, and bone. The GIP pathway appears to enhance insulin sensitivity independently, improve fat oxidation, and amplify GLP-1's appetite-suppressing effects through central nervous system signaling. Tirzepatide is FDA-approved as Mounjaro (for type 2 diabetes) and Zepbound (for chronic weight management).

The key distinction: CagriSema pairs a GLP-1 agonist with an amylin analog. Tirzepatide pairs GLP-1 agonism with GIP agonism. Both are dual-pathway strategies. Tirzepatide: The SURMOUNT Trial Program. SURMOUNT-1, published in the New England Journal of Medicine in 2022 (PMID: 35658024), enrolled 2,539 adults with obesity or overweight without diabetes. At 72 weeks, participants on tirzepatide lost an average of 15.0% (5 mg), 19.5% (10 mg), and 20.9% (15 mg) of their body weight. Using an adherence-adjusted estimand, the 15 mg group achieved 22.5% average weight loss. More than one-third of participants on the highest dose lost over 25% of their body weight.

SURMOUNT-2 (PMID: 37840095) confirmed strong results in patients with obesity and type 2 diabetes: 14.7% weight loss on 15 mg at 72 weeks. SURMOUNT-3 and SURMOUNT-4 examined combinations with intensive lifestyle therapy and treatment withdrawal, respectively. The pattern across all SURMOUNT trials is consistent: tirzepatide at maximum dose produces roughly 20-26% weight loss in obesity populations, a level previously only achievable through bariatric surgery.

The SURPASS trials established tirzepatide's efficacy in type 2 diabetes. SURPASS-2 (PMID: 34170647) directly compared tirzepatide to semaglutide 1 mg and showed superior A1C reduction and weight loss at all three tirzepatide doses. That trial used the diabetes dose of semaglutide (1 mg), not the obesity dose (2.4 mg), which limits the comparison -- but the superiority was clear within that study design.

CagriSema: The REDEFINE Trial Program. CagriSema's Phase 3 clinical program is called REDEFINE. The data published so far comes primarily from Phase 2 trials and early Phase 3 readouts.

The most cited Phase 2 study was published in The Lancet in 2023 (PMID: 37352883). It enrolled adults with type 2 diabetes and randomized them to CagriSema, cagrilintide alone, semaglutide alone, or placebo. At 32 weeks, CagriSema produced 15.6% weight loss in patients with type 2 diabetes. For context, semaglutide alone produced 5.1% in the same trial, and cagrilintide alone produced 8.1%. The combination outperformed both individual components by a significant margin. This is notable because the 15.6% figure was achieved in a diabetes population (which typically shows less weight loss than non-diabetes populations) and at only 32 weeks (shorter than the standard 68-72 week obesity trial endpoint).

The REDEFINE 1 trial -- the first pivotal Phase 3 study in adults with obesity without diabetes -- reported topline results in late 2024. Novo Nordisk announced that CagriSema achieved approximately 22.7% mean weight loss at 68 weeks. This placed it numerically in the same range as tirzepatide 15 mg in SURMOUNT-1 (20.9-22.5%, depending on estimand). However, REDEFINE 1 is a placebo-controlled trial, not a head-to-head against tirzepatide. Cross-trial comparisons are inherently imprecise due to differences in population characteristics, sites, and methodology.

REDEFINE 2, studying CagriSema in adults with obesity and type 2 diabetes, is ongoing. Several additional REDEFINE trials are examining cardiovascular outcomes, adolescent populations, and other subgroups. If the full program succeeds, Novo Nordisk is expected to file for FDA approval, with a potential approval date in late 2026 or 2027.

The head-to-head question. As of this writing, no published randomized trial has directly compared CagriSema to tirzepatide in the same patient population, at the same time points, using the same endpoints. Novo Nordisk has announced the REDEFINE 6 trial (also referred to in some registries as a superiority study), which is designed to compare CagriSema directly against tirzepatide. Until that data is published, any claim that one is definitively "better" than the other for weight loss is premature.

What we can say: based on available data, both therapies appear capable of producing 20%+ mean weight loss in obesity populations without diabetes. The confidence interval around that comparison is wide enough that neither drug has a clear, proven advantage over the other on weight loss alone.

Cardiovascular outcomes: the data gap. Neither CagriSema nor tirzepatide (specifically for obesity) has a completed cardiovascular outcomes trial comparable to semaglutide's SELECT trial (PMID: 37952131), which showed a 20% reduction in major adverse cardiovascular events. The SURPASS-CVOT trial for tirzepatide in type 2 diabetes is ongoing. For CagriSema, cardiovascular outcomes data will come from the REDEFINE program but will take years to mature.

This matters for patients with established heart disease. Semaglutide (a component of CagriSema) has proven cardiovascular benefit as a standalone drug. Whether combining it with cagrilintide alters that cardiovascular profile -- positively or negatively -- is unknown. Side Effects and Tolerability

Both CagriSema and tirzepatide produce gastrointestinal side effects characteristic of incretin-based therapies. The patterns are familiar, but the nuances between these two combinations may matter for individual patients.

Nausea and GI effects remain the dominant concern. In tirzepatide's SURMOUNT-1 trial, nausea rates ranged from 24.6% to 33.3% across dose levels. Vomiting occurred in 8.3-12.2%. Diarrhea in 17-21%. Constipation in 11-17%. These rates are somewhat lower than those reported for semaglutide 2.4 mg alone in the STEP trials, where nausea hit 44.2%. The GIP component of tirzepatide is believed to partially buffer the GI disturbance caused by GLP-1 agonism.

CagriSema's GI profile is still being characterized. In the Phase 2 Lancet study (PMID: 37352883), GI adverse events were common but broadly similar to the semaglutide-alone arm. Nausea occurred in approximately 28% of CagriSema participants, and most cases were mild to moderate. Early Phase 3 data from the REDEFINE program has not suggested any unexpected safety signals beyond what is typical for GLP-1-based therapies.

There is a theoretical question about whether the addition of amylin agonism compounds GI side effects. Both GLP-1 and amylin slow gastric emptying. Two signals telling the stomach to slow down could, in theory, produce more pronounced gastroparesis-like symptoms (bloating, early satiety, nausea) than either signal alone. Commercial savings programs from Eli Lilly can reduce out-of-pocket costs significantly for eligible patients. Market dynamics -- including formulary negotiations, payer pressure, and competition from tirzepatide -- will influence the final number.

The Novo Nordisk vs Eli Lilly dynamic. This comparison is as much a corporate rivalry as a clinical one. Novo Nordisk dominates the GLP-1 market through semaglutide (Ozempic, Wegovy, Rybelsus). Eli Lilly captured significant market share with tirzepatide (Mounjaro, Zepbound) by demonstrating superior weight loss. CagriSema is Novo Nordisk's strategic response -- an attempt to reclaim the "best weight loss drug" title by adding amylin to their existing GLP-1 franchise. Tirzepatide is available, has strong clinical data, and is producing meaningful results for patients right now. Delaying treatment to wait for a drug that may or may not be approved on schedule is a decision that should be made with your physician, not based on headlines about clinical trial readouts.

The Bottom Line

CagriSema and tirzepatide represent two different answers to the same question: how do you improve on semaglutide alone? Tirzepatide adds GIP agonism. CagriSema adds amylin agonism. CagriSema is the one to watch. If the REDEFINE program delivers on its early promise and the head-to-head data against tirzepatide (REDEFINE 6) demonstrates superiority or equivalence, CagriSema could meaningfully reshape the competitive landscape.

For patients already responding well to tirzepatide, there is no evidence-based reason to switch to CagriSema even after approval unless the head-to-head data shows a clinically meaningful advantage. For patients who do not respond adequately to tirzepatide or experience intolerable side effects, CagriSema's different secondary mechanism (amylin vs GIP) offers a biologically plausible alternative -- though that clinical scenario has not been formally studied.

The next 12-18 months will be decisive. REDEFINE pivotal data, the REDEFINE 6 head-to-head, and the FDA regulatory process will collectively determine whether CagriSema joins tirzepatide as a first-line option or occupies a more specialized role. Until then, this is a comparison between a proven therapy and a promising one. Both deserve attention. Only one is ready for your medicine cabinet.
